Blackstone Health, Inc. is a non-profit organization managed by the VNA of Care New England which facilitates two major programs in the State of Rhode Island: The New Horizons Adult Day Center and the Blackstone Health Café. The former provides services to adults who suffer from some form of physical or cognitive disability, while the latter is comprised of 17 nutrition sites throughout the state wherein meals and nutrition education are provided to individuals 60 years of age or older.
Clients should choose us because we are a viable alternative to a long-term care facility. In short, we venture to maintain the integrity of the family unit by way of providing essential services that would allow frail adults to live at home longer than would normally be expected without our services and respite program for caregivers.
The New Horizons Adult Day Center is currently confined to one location only. Services include, but are not limited to: Health Monitoring, Medication Management, Case Management, Continuous Supervision, Nutritious Meals, and Planned Activities and Crafts.
A warm greeting accompanied by a complimentary tour of our facility. If interest in our services were to prevail post-tour, we would welcome said patient or caregiver to stay for an admissions meeting wherein facility regulations, standards, and intake procedures would be discussed.
This program provides balanced and nutritional meals to disabled adults and seniors in a community-dining setting. There are 16 Blackstone Health Café dining locations across the state of Rhode Island. Participation in the dining program is available to individuals who are at least 60 years of age and their spouse, as well as persons with a disability under 60 years of age. Hours of operation vary by dining location.
