Dear Valued Client,

I am writing to you today because I wanted to personally inform you of some important changes in the HealthTouch private duty home care service you receive. Despite significant staffing challenges and a decline in the number of our home care clients over the last three years, the staff at HealthTouch have continued to provide you with important care and support services.

Today, I regret to inform you that effective April 15, 2017, HealthTouch will no longer be able to provide your home care. This is not a decision that was taken lightly. I understand the importance of the sustainability of your care and I know, in many instances, you have developed close bonds with your care provider and have welcomed them in as family.

As we plan for this change, your ongoing care plan is of great importance to me. Should you have a preferred provider to assume services, please know you are encouraged to make that transition as soon as you are able. You do not need to wait until April 15 and for your planning purposes I would encourage this. I understand you probably have a number of questions and concerns. In closing, I want to thank you for the trust you have put in HealthTouch, allowing us to care for you or your loved ones. I regret the need to share this information today but do wish you well for the future.
