Care Management
Under the guidance of a Registered Nurse Care Manager, a specialized team can assist you with all aspects of long-term care, enabling our clients to maintain their safety, security, and independence at home. Following an initial comprehensive needs assessment, the team which may include a nurse, certified nursing assistants, and/or social workers will collaborate with the client and family to establish a mutually agreed upon, affordable care management plan.
Medication Management
Registered nurses from HealthTouch can prefill medication cassettes, review administration and dosage, and coordinate with the client’s doctor if needed.
Assistance with Bill Paying/Budgeting
Help is available with bill paying, budgeting or checkbook balancing. We can also handle issues with medical insurance companies.
Transition to Alternative Living Arrangements
When a transition to an alternate living arrangement is preferred, our care manager will identify appropriate care settings, present these to the family or caregiver, and help ensure a smooth and timely transition.
Lifeline® Personal Emergency Response System

Personal Care/Assistance with Activities of Daily Living
Daily life is made easier and safer with help bathing, dressing, and getting ready for bed. Certified nursing assistants can help with all aspects of personal care including bathing and grooming, whether the need is for a full bath, sponge bath, or just a shampoo.
Light Housekeeping
HealthTouch staff can do the laundry, assist with general tidying of the home, or help with home or yard maintenance.
Meal Preparation
HealthTouch staff can do grocery shopping and prepare meals according to dietary needs for breakfast, lunch and/or dinner.
Newborn Care/Doula Services
Meeting the needs of a newborn can be challenging, for both a new mom and the entire family. New mothers, their infants and their families can receive physical care and emotional support from trained postpartum doulas. Doulas specialize in breastfeeding support, care of multiples, sibling adjustment, and if needed, advice on community resources.
Private Pay Nursing Care
Registered nurses and certified nursing assistants can provide one-on-one personalized care for patients during their hospital stay, at an assisted living facility, at a skilled nursing facility, or even within the comfort of their own home.
Transportation
HealthTouch staff will transport you to doctors’ appointments, grocery shopping, or to help you with any other errands.
Wake-Up and Tuck-In
Certified nursing assistants can help you to get out of bed in the morning and get dressed for the day, or visit you at night to help you safely to bed.
