Phase I (0 to 3 Months)

  • Focus on orienting to the agency and care of the home care patient.

Phase II (4 to 6 Months)

  • Focus on managing a caseload as a team.

Phase III (7 to 12 Months)

  • Focus on becoming independent case managers and developing professionally
