Care Management is offered by HealthTouch, an affiliate of VNA of Care New England. The goal of Care Management is to develop a personalized plan of care to help older adults maintain their health, well-being and quality of life, so they may, whenever possible, remain in their community, living safely and comfortably in their home, where they want to be.
However, as gradual or sudden changes in health status occur, everyday activities such as bathing, dressing, and getting out of bed can become more difficult. Feelings of lost independence grow as transportation options decrease. Daily tasks of housekeeping, shopping and meal preparation may pose significant challenges.
Extra support is often needed for older adults to continue living both safely and comfortably in their homes amid familiar surroundings. HealthTouch can help.
Services offered and/or coordinated by the Care Management Program include:
- Assistance with activities of daily living.
- Medication monitoring.
- Daily telephone check-ins.
- Meal preparation/shopping.
- Assistance with bill paying/budgeting.
- Transition to alternative living arrangements.
In our mobile society, family members often live far away and cannot help their loved ones as much as they want to. Even for those who do live nearby, the demands of juggling a family, career and the needs of an aging family member can be overwhelming.
Lawyers, financial planners or other interested parties may identify areas of concern, but do not always have available resources to coordinate multiple services for their elderly clients that may include extensive, ongoing and advanced communication and coordination among family members, physicians and other health care providers.
When a transition to alternate living arrangements is preferred, we help identify appropriate care setting options, which are presented to the family or caregiver to assist in ensuring a smooth and timely transition.

Our team of specially trained specialists consists of professional care managers, certified nursing assistants, homemakers and companions who adhere to the highest standards of quality and professionalism. Leading the team is a certified geriatric care manager, a registered nurse who has received advanced training and certification in the care, treatment and special needs of older adults.
In addition, our home health aides not only receive the finest training, but are also insured, bonded and closely supervised by the nursing staff at HealthTouch. HealthTouch is a member of the National Association of Professional Geriatric Care Managers and accredited by the Joint Commission, a national mark of quality and excellence

Following the needs assessment and in collaboration with all interested parties, a mutually agreed upon, affordable Care Management Plan is established that allows for changing circumstances and individual needs. Once designed and agreed upon, the Care Management team implements and monitors the care plan. Comprehensive, real-time feedback and communication is provided to designated family members or caregivers, whether they reside in the next town or across the country, providing unparalleled peace of mind.
